An employee was cooking a batch of lubricating grease. Flames and grease slurry came out of the grease cooking kettle and splashed onto the employee; this also started a fire outside the kettle. The grease/oil slurry was approximately 185-200 degrees and caused second degree burns to the employee's face, neck, arms and hands.

An employee was spraying out glue pans and machine side frames with a water hose. As the employee used a brush on the glue roll as it was turning, his glove and the tip of his right ring finger were caught and pulled between the glue roll and the anti-wrap bar. He suffered a fingertip amputation.

An employee was manually turning over a dock plate that was sitting flat on the lifts of a forklift truck in preparation for installation. The dock plate consisted of two metal pieces connected by a hinge. Three of the employee's right-hand fingertips were caught and amputated in the hinge.

As he put his hand out to brace the fall, it was caught between the motor plate and a pallet. His left middle and ring fingertips were amputated.
